Hospital to home care offers a unique opportunity in the recovery process of an individual. Hospital to home care is provided to someone experiencing an illness outside the confines of an acute care hospital and integrates caregivers and families within their own home.
One of the leading causes of hospital readmission or slow post-hospitalization recovery is the lack of proper support following a hospital discharge.
So whether you are transitioning directly home after a hospitalization or moving through the care continuum via rehabilitation or medical care facility, being cared for in your own home plays a vital role in your safety and successful recovery process.
OPSCare can help bring that change back with the help of our on-site professionals (Occupational Therapists). OPSCare staff are trained to help with some of your discharge checklist that you may find difficult at first, and can slowly assist in enabling your independence again, by helping you settle back in to your familiar surrounding
Our post-operative care is usually planned in advance of your hospital admission. This ensures that every element of your care package is organised to commence from the moment that you are discharged from hospital.
We can liaise with the hospital Discharge team to ensure that any required equipment has been installed in your home, for example hoisting equipment. In addition, we will ensure that a full risk assessment is carried out in your home to make sure it is completely safe for your return.
As part of our hospital to home service, we will support you with your transfer from hospital, making sure you feel comfortable at all times. We can arrange for an Occupational Therapist or Support Worker to meet you at the hospital and, where required, can organise transport on your behalf.
We understand that hospitals are not always able to give substantial notice before arranging your discharge. Should you need us to respond quickly, our service can often commence within 24 hours of your initial enquiry. Our Care at Home Team can liaise and work closely with your G.P, hospital Doctors, Occupational Therapist, Nurses, Discharge Teams and Social Workers to ensure you have a smooth transition from hospital to home.
